TOOL KITS

A series of tool kits have been developed to support the implementation of evidence-based guidelines into busy primary care practices. These tools may assist you with the integration and organization of daily activities for the screening and secondary prevention of cardiovascular diseases and risk factors into your practice. In addition, there are some tools which may assist your patients with the self management of their disease. The majority of the tools are in English but there are a few in French that may be accessed by clicking on the link to the French page. All of the tools, either in English or in French, may be adapted to the needs of your own practice.
