A Resource for Primary Care Physicians in the Champlain District

The Champlain Cardiovascular Disease Prevention Network, in collaboration with its health and community partners, has developed the Champlain CVD Prevention & Management Guideline to facilitate evidence-based, primary and secondary preventive care. The guideline is intended to serve as a desktop resource for primary care practitioners in our region. It has been tailored to the Champlain District by including specialty and community-based resources which are relevant to the prevention and management of cardiovascular disease.

More than 45 local specialists and family physicians have supported the Guideline’s development. 

The Champlain District is home to many of Canada’s foremost experts in the prevention and management of heart disease, stroke, and diabetes.  The Champlain Primary Care CVD Prevention and Management Guideline represents the combined efforts of more than 45 specialists and community-based practitioners from across our region. Each section of the guideline was prepared by one of nine  evidence-monitoring committees.  Each evidence monitoring committee reviewed the latest evidence and summarized the most relevant strategies for primary care practitioners. The guidelines reflect the best available knowledge and local opinions on screening, prevention, and management of heart disease, stroke, diabetes and the associated risk factors.  Committees will meet to review annually the guidelines contents to ensure they reflect the best available evidence. 

 

The guideline is divided into three parts:

Part 1

Screening for CVD Risk Factors;

Part 2

Risk Factor Management (Hypertension, Cholesterol, Chronic Kidney Disease, Smoking Cessation, Physical Activity, Obesity/ Weight Management); and,

Part 3

Disease Management (TIA/ Stroke, CAD/ PVD, Diabetes). 


Each section includes a summary and/or decision tree of current, practical, and evidence-based strategies, supplemental information (where appropriate), and links to relevant, local resources to assist patients in accessing supports within their own community for health education and secondary prevention.
